* Policy is Guaranteed issue to persons age 65 and older – based on age at last birthday. Spouse of any individual that qualifies may apply regardless of age.

* All policies become effective on the first day of the calendar month that next follows.

* Applicant can choose Standard Plan (low benefit) with a $1,000 max annual benefit or Enhanced Plan (high benefit) with a $1500 max annual benefit.

* Choice of $25,$50,$75 or $100 deductible

Type III Services – Biopsies, hisopathologic examinations;repairs to existing crowns, dentures and bridges;recemenation of existing crowns, inlays, onlays, and bridges; therapeutic drug injections.

Type IV Services – Endodontics (root canals); non-surgical periodontics (gum treatment); surgical extractions; extraction of impacted teeth; other oral surgery; general anesthesia and IV sedation; denture relines and rebases; denture adjustments.

Type V Services – Periodontal Surgery; inlays and onlays; crowns; complete and partial dentures; fixed bridgework
